Annual Registration & Enrollment Notice

A current, signed registration and consent form is required for participation in Urban Impact programs each program/school year. Prior registrations do not remain valid.

Program enrollment is subject to availability. Submission of this form does not guarantee immediate placement. If a program is at capacity, your child may be placed on a waitlist, and you will be notified if space becomes available.
